Missed Signs
From the June 27, 1951, edition of The Sporting News:

Dutch Leonard, ace relief pitcher of the Cubs, ran into difficulties with too many Edwardses and a change of signals, June 16. Facing Hank Edwards of the Dodgers in the ninth inning, Dutch thought catcher Bruce Edwards, recently acquired from Brooklyn, had signalled him to throw a fastball, which Hank hit for a double. Instead, Bruce had asked for a curve. Leonardhad forgotten the signals had been changed because of four Cubs having joined the Dodgers.
